We work as a team! Although Anatt is the main contact, she is not a sole practitioner. She has her business partner, Elaine, who is also a seasoned Care Manager and member of ALCA. There is always someone available to assist you! In addition, we have a third Care Manager, a visiting LVN, to respond at any time of day.

We improve communication between medical professionals, and will be your “hub” of information as we communicate with everyone involved. We attend physician visits and advocate appropriately when necessary.

We want you to talk to us! We do not charge for phone calls if they are of informational nature. If you have a question or concern, we want you to call us so that we can be aware of all issues and address them quickly.

We have lessened, and in some cases, completely eliminated frequent hospital visits for clients by effectively managing medication distribution, reducing hazards or fall risks, and notifying doctors of changes in conditions, so that the doctor sees the patient before a hospital visit is necessary.
